Welcome to our free no-obligation "Approved Forever Home" application form. All prospective Pup owners must fill out this application. It's very easy and the information will help the both of us learn what Pup will be just right for you and your family. You might find some questions/dilemmas you haven't thought about yet. Please answer all questions and be as truthful as possible. You are under no obligation to purchase a Pup by completing this application. Being an approved 'forever home' does not guarantee you a Pup. Please see the Waiting list page and store page for more details. Please read through my website pages before emailing questions. I've worked hard to answer most questions including pricing, shipping, Pup availability, health and rearing.

*As an adult, have you ever given away, gotten rid of, lost, or turned in a dog to a pound? Things do happen, please explain.

*Do you rent or own? Apartment/condo or house?

*Will your landlord let you have a dog? Please verify before purchasing any puppy.

*Do you have a backyard and is it secure for a little dog?

*Do you move or travel often and who will take care of Pup?

*Do you plan to spay/neuter your Pup?

*Will you require your new Pup to be shipped to you? If yes, please list nearest major airport to you and your travel time to that airport to pick up Pup.

*What is the temperament of the new Pup you desire? Alpha dog, lap dog, energetic, hyper, brave, meek, single dog or multi dog home, etc.

*Do you plan on attending any puppy training classes?

*Where will your Pup sleep?

*Will your Pup be alone during the day? Please estimate how many hours and include where will it stay: crate, loose, bathroom, etc.?
